Go with the size matching your bust measurement, since it's easier to take in a waist than let out a bust. Consider custom stitching if the gap is large.
A well-fitted blouse should allow you to raise your arms and breathe comfortably — roughly half an inch of ease is normal, not more.
Gaping at the bust or back, tight armholes, shoulder seams sliding off, or a hemline that rides up are all signs it's time to size up or down.
